Enter a serial number for an active phone

Click continue as shown

Make the selection shown and click continue

Click No

Enter the zip code. If you want a sim card for a carrier other than the one you currently have use an appropriate zip code.

Enter your shipping information.

Confirm your shipping information and continue

You should receive a new sim card within 3 business days. It usually takes 2 days. Your phone will remain active.
